Price is for appointment only. Notary fees of $10 Per Notarial Act (seal) are applied separately. Travel Fees may apply depending on location. *The patient must be aware and alert, able to understand the content and purpose of the document and have the ability to sign the document of their own will, free of any mind-altering substances. If the client is unable to meet these requirements, the signing cannot be completed and an attorney should be contacted.* All clients must posses one of the following types of acceptable Identification: State-issued driver's license State-issued identification card U.S. passport issued by the U.S. Department of State U.S. military ID State, county and local government IDs Permanent resident card, or green card Inmate ID issued by the State Department of Corrections or Federal Bureau of Prisons, if the inmate is in state or federal custody Inmate ID issued by a county sheriff, if the inmate is in the custody of the county sheriff *A 48-hour notice is required if a witness is requested through Better Days Notary & Signing Agent, LLC.
